Dr. Boles is not a member of any third-part contracts, meaning that he is “out-of-network” for all payers. This is a “cash practice”. It is anticipated that most PPO patients will be reimbursed a proportion of their payment, dependent on your insurance plan, deductible, and other factors. Upon request, Dr. Boles will provide you with the documents (super bill and detailed consultation note) needed for you to apply for reimbursement from your insurance company. Additionally, Dr. Boles does not accept government plans such as MediCal/Medicaid, Medicare, and California Children’s Services (CCS).
